Pay: $58–$65 per hour, based on experience

Care Connection Rehabilitation Center is seeking a compassionate and skilled Physical Therapist (PT) to join our interdisciplinary rehabilitation team. This PRN opportunity is ideal for a PT who wants to make a meaningful impact while providing high-quality care in a supportive skilled nursing environment.

Responsibilities

• Evaluate patients and assess physical conditions, mobility, and functional limitations

• Develop and implement individualized treatment plans

• Provide therapeutic exercises, stretching, and hands-on treatment

• Monitor patient progress and modify treatment plans as needed

• Document evaluations, treatments, and patient progress accurately

• Educate patients and families on therapy goals, home exercises, and recovery

• Collaborate with nursing and other members of the interdisciplinary care team

Qualifications

• Doctor of Physical Therapy (DPT) from an accredited program

• Current New Jersey Physical Therapist license

• Minimum 2 years of experience preferred

• Skilled nursing or nursing home experience preferred

• Strong clinical assessment, communication, and interpersonal skills

• Familiarity with SigmaCare and Net Health preferred
