About the Role:

As an LPN Licensed Practical Nurse, you will be responsible for providing high-quality patient care to our residents, including:

• Assisting with medication management and administration

• Monitoring vital signs and reporting changes to the nursing team

• Providing wound care and dressing changes as needed

• Assisting with activities of daily living (ADLs) such as bathing, dressing, and grooming

• Developing and implementing individualized care plans

• Collaborating with the interdisciplinary team to ensure quality patient care

Responsibilities:

• Provide direct patient care, including administering medications, performing treatments, and providing assistance with ADLs

• Participate in the development and implementation of individualized care plans

• Monitor and report changes in patient condition to the nursing team

• Maintain accurate and detailed records of patient care and treatment

• Collaborate with the interdisciplinary team to ensure quality patient care

• Participate in continuing education and professional development opportunities to stay current with industry best practices
