About the Role:

As a Physical Therapist at Spring Creek Healthcare Center, you will have the opportunity to make a meaningful impact on the lives of our patients and residents. You will work collaboratively with our interdisciplinary team to develop and implement individualized treatment plans that promote optimal physical function, mobility, and independence. Our state-of-the-art rehabilitation centers, equipped with the latest technology and equipment, provide a supportive environment for patients to achieve their goals.

Responsibilities:

• Develop and implement individualized treatment plans for patients and residents with a wide range of physical impairments and disabilities

• Conduct initial and ongoing evaluations, assessments, and interventions to achieve optimal outcomes

• Collaborate with our interdisciplinary team, including physicians, occupational therapists, and speech-language pathologists, to ensure comprehensive care

• Provide education and training to patients, families, and caregivers on physical therapy techniques and exercises

• Maintain accurate and timely records of patient progress and treatment plans

• Stay current with industry developments and best practices, participating in Continuing Education opportunities and professional development activities
