SYNERGY HomeCare is looking for an outgoing, motivated Community Liaison to help grow our non-medical home care business by building relationships with healthcare and senior care professionals throughout our local communities.
This is a field-based business development role, not a desk marketing position. You will spend much of your time visiting assisted living communities, senior living facilities, skilled nursing facilities, rehabilitation centers, RCFEs, home health agencies, hospitals, physician offices, and other potential referral partners.
Your goal is to build trusted relationships, create awareness of SYNERGY HomeCare, and generate qualified opportunities from families who need non-medical home care.
Visit local senior living and healthcare organizations on a regular basis
Build relationships with social workers, discharge planners, case managers, administrators, nurses, care coordinators, and other referral partners
Introduce SYNERGY HomeCare and explain our non-medical home care services
Identify families and seniors who may need assistance at home
Follow up consistently with referral partners and prospective clients
Attend networking events, community events, professional meetings, and senior-focused events
Develop new referral relationships while maintaining existing relationships
Track visits, contacts, referrals, and follow-up activities
Work closely with our office team to help referred families receive timely support
Represent SYNERGY HomeCare professionally throughout the community
What We're Looking For
Previous experience in home care, home health, hospice, senior living, healthcare, medical sales, outside sales, senior placement, or business development is preferred, but we are open to strong sales-oriented candidates from other industries.

Compensation
Hourly base pay
Performance-based commissions

Valid driver's license
Reliable vehicle and current auto insurance
Comfortable driving throughout the assigned territory

Basic ability to track leads and activities using a CRM or similar system
This role will focus primarily on communities throughout the San Fernando Valley and Ventura County, including areas such as Chatsworth, Porter Ranch, Northridge, Woodland Hills, West Hills, Simi Valley, Thousand Oaks, Westlake Village, Oak Park, and surrounding communities.
