Position: Social Worker

Schedule: Full-Time
Salary: $68,000–$72,000 annually

About the Role

Union Plaza Care Center is seeking a Full-Time Social Worker to support residents and families by providing psychosocial support, assisting with care and discharge planning, coordinating community resources, and advocating for residents' needs.

Bilingual proficiency in English and Korean is preferred but not required. Candidates who are fluent in Korean are strongly encouraged to apply.

Key Responsibilities

• Conduct resident and family interviews and assessments.

• Develop and participate in individualized care plans.

• Assist with discharge planning, referrals, transportation, and community resources.

• Support residents and families with Medicare, Medicaid, and financial assistance resources.

• Collaborate with physicians, nursing, rehabilitation, and other departments.

• Maintain accurate documentation and complete Social Services/MDS requirements.

• Assist with MOLST/POLST and advance directive documentation.

• Participate in care plan meetings, quality assurance activities, and facility surveys.

• Advocate for residents and promote their emotional, social, and psychological well-being.

Requirements

• Korean language proficiency preferred, but not required.

• Minimum 1 year of supervised social work experience in a healthcare setting.

• Bachelor's degree in Social Work or a related field such as Sociology, Psychology, Rehabilitation Counseling, or Special Education.

• MSW preferred.

• Nursing home/long-term care experience preferred.

• Strong communication, organizational, and interpersonal skills.

• Ability to work effectively with residents, families, and an interdisciplinary team.
