Remote Nurse Practitioner

We are seeking a Family Nurse Practitioner (FNP) to join our team on a fully remote, per diem basis providing telehealth care to patients throughout New York.

Responsibilities

• Conduct telehealth patient evaluations

• Assess and manage acute and chronic medical conditions

• Review medications and medical history

• Develop appropriate treatment plans

• Order referrals and services when medically necessary

• Complete accurate and timely clinical documentation

• Coordinate with our clinical and administrative team

Requirements

• Active New York State Nurse Practitioner license

• Family Nurse Practitioner (FNP) certification

• Medicaid enrolled and eligible to bill

• Strong communication and telehealth skills

• Comfortable working independently in a fast-paced environment
