Mountainside Nursing Home is seeking a dedicated and compassionate Physical Therapist to join our team of healthcare professionals in Mountainside, New Jersey. As a Physical Therapist at Mountainside Nursing Home, you will play a vital role in providing high-quality rehabilitation services to our long-term care residents.

About the Role:

As a Physical Therapist - Long Term Care, you will be responsible for evaluating, planning, and implementing individualized physical therapy programs to help our residents achieve their maximum functional potential. You will work closely with our interdisciplinary team to develop and implement treatment plans that address the unique needs and goals of each resident.

Responsibilities:

• Conduct thorough evaluations of new and existing residents to identify rehabilitation needs and establish goals

• Develop and implement individualized physical therapy programs, including goal setting, treatment planning, and documentation

• Provide direct patient care, including manual therapy, exercise programming, and education sessions

• Collaborate with other healthcare professionals, including occupational therapists, speech therapists, and nursing staff, to ensure comprehensive care

• Monitor and re-evaluate patient progress, making adjustments to treatment plans as needed

• Maintain accurate and up-to-date records of patient care and treatment

Requirements:

• A Doctor of Physical Therapy (DPT) degree from an accredited program

• Current physical therapy license in the state of New Jersey

• Experience working in a long-term care setting is preferred, but not required
