In this position, you will provide personalized support based on each individual’s abilities, preferences, and care needs. Services may be provided in private homes, community settings, group homes, assisted living facilities, or day programs.

Primary Responsibilities

Depending on the individual’s plan of care, your responsibilities may include:

• Assisting with bathing, dressing, grooming, and incontinence care

• Providing mobility and transfer support

• Preparing meals and completing light housekeeping or laundry

• Offering companionship, encouragement, and advocacy

• Providing transportation and community support

• Giving medication reminders

• Helping individuals participate in daily activities
