Northcrest Nursing & Rehabilitation Center is seeking a compassionate, organized, and resident-focused Social Services Director to join our leadership team. This position plays an important role in supporting our residents and families while helping ensure a positive experience from admission through discharge.

Key Responsibilities:

• Lead and oversee the facility’s Social Services department

• Advocate for residents and support their psychosocial and emotional needs

• Build strong relationships with residents, families, and responsible parties

• Participate in care planning and interdisciplinary team meetings

• Coordinate discharge planning and safe transitions of care

• Connect residents and families with community resources and services

• Address resident and family concerns promptly and professionally

• Complete required assessments, documentation, and care plan updates

• Support resident rights and regulatory compliance

• Collaborate with Nursing, Admissions, Therapy, Activities, and facility leadership

Qualifications:

• Degree in Social Work, Human Services, or a related field preferred

• Previous social services experience in skilled nursing, long-term care, healthcare, or a related setting preferred

• Knowledge of resident rights and discharge planning is a plus

• Strong communication, organization, and problem-solving skills

• Compassionate, professional, and team-oriented
