Homeside Rehab is seeking compassionate and dedicated Occupational Therapists (OTs) and Certified Occupational Therapy Assistants (COTAs) to provide individualized, evidence-based occupational therapy services to patients in their homes throughout the Bronx.

Responsibilities

• Perform comprehensive occupational therapy evaluations (OTs) and develop individualized treatment plans based on each patient's needs and goals.

• Provide evidence-based occupational therapy interventions to improve fine motor skills, upper extremity function, activities of daily living (ADLs), sensory processing, coordination, and functional independence.

• Deliver skilled therapy services in patients' homes while promoting improved function and quality of life.

• Complete accurate, timely, and compliant electronic documentation.

• Collaborate with physicians, caregivers, and the interdisciplinary healthcare team to coordinate patient care.

• Educate patients and caregivers on home exercise programs, adaptive equipment, and therapeutic strategies.

• Communicate effectively with office staff regarding scheduling, patient updates, and clinical needs.

Qualifications

• Active New York State Occupational Therapist (OT) or Certified Occupational Therapy Assistant (COTA) License (Required)

• Minimum 1 year of experience in Home Health, Outpatient, Early Intervention, or Skilled Nursing Facility (SNF) preferred

• Excellent clinical assessment and communication skills

• Ability to work independently in a home health setting

• Basic computer proficiency for electronic documentation

• Valid New York State driver's license and reliable transportation

Position Details

• Job Types: Full-time, Part-time, Per Diem

• Compensation: $75.00–$100.00 per hour

Schedule

• Choose your own hours

• Monday through Friday

• Day and evening shifts available

• Weekend availability optional

Cases are assigned based on your preferred coverage area.
