Responsibilities

• Evaluate the patient and develop an individualized plan of care based on clinical needs and goals

• Provide skilled speech-language therapy services in the patient's home

• Provide evidence-based treatment for speech, language, articulation, fluency, voice, and related communication needs

• Complete accurate and timely clinical documentation

• Communicate effectively with the patient, caregivers, office staff, and clinical team

• Coordinate care with other healthcare professionals as appropriate

Qualifications

• Active New York State Speech-Language Pathologist (SLP) License

• Strong clinical and interpersonal communication skills

• Ability to work independently in a home-based setting

• Basic computer skills for documentation and communication

• Home health, outpatient, pediatric, or skilled nursing experience preferred

• Reliable transportation for home visits
