We are seeking a highly dedicated and experienced Director of Rehabilitation to join our team at Hope Creek Nursing and Rehabilitation Center in East Moline, Illinois. As a key leader in our organization, the Director of Rehabilitation will play a vital role in developing and implementing comprehensive rehabilitation programs for our patients.

About the Role:

• The Director of Rehabilitation will oversee the day-to-day operations of our rehabilitation department, including supervising a team of rehabilitation professionals

• Develop and implement individualized rehabilitation plans for patients, focusing on achieving optimal functional status and independence

• Collaborate with other healthcare professionals to ensure a multidisciplinary approach to patient care

• Participate in quality assurance and performance improvement activities to ensure the delivery of high-quality patient care

Responsibilities:

• Develop and implement rehabilitation programs that meet the unique needs of our patients

• Supervise and support a team of rehabilitation professionals, including occupational therapists, physical therapists, and speech therapists

• Conduct patient assessments to identify rehabilitation needs and develop individualized plans of care

• Foster collaborative relationships with other healthcare professionals, including physicians, nurses, and social workers

• Monitor and improve rehabilitation program outcomes, including patient satisfaction and discharge outcomes

Requirements:

• Master's degree in Rehabilitation Counseling, Occupational Therapy, Physical Therapy, or related field

• Strong clinical and leadership skills

• State licensure as a rehabilitation professional

• Certification in a rehabilitation specialty
