Responsibilities:

• Provide direct patient care services, including vital signs monitoring, wound care, and medication administration, under the supervision of a licensed nurse

• Develop and implement comprehensive care plans, in collaboration with interdisciplinary team members, to meet the unique needs of each resident

• Participate in quality improvement initiatives and staff development programs to continually improve patient outcomes and staff performance

• Demonstrate a strong commitment to patient-centered care, empathy, and compassion, while maintaining a clean and organized work environment

• Collaborate with other healthcare professionals to ensure seamless transitions and continuity of care

Requirements:

• Current LPN licensure in the State of New Jersey

• Graduation from an accredited LPN program

• Ability to work effectively in a fast-paced environment, prioritizing multiple tasks and responsibilities
