We are seeking an experienced and compassionate Wound Care Director to oversee the assessment, prevention, treatment, and ongoing management of wounds across our skilled nursing facilities. The Wound Care Director will work closely with nursing leadership, physicians, nurse practitioners, therapists, dietitians, and facility staff to ensure high-quality, evidence-based wound care and positive resident outcomes.

Key Responsibilities:

• Oversee and coordinate wound care programs across assigned facilities

• Conduct comprehensive wound assessments and develop individualized treatment plans

• Monitor wound healing progress and revise interventions as clinically appropriate

• Collaborate with physicians, advanced practice providers, nursing staff, dietitians, and other members of the interdisciplinary team

• Ensure accurate and timely wound documentation in accordance with facility and regulatory requirements

• Monitor wound-related clinical outcomes and identify opportunities for improvement

Qualifications:

• Current LPN-RN license in the State of Illinois

• Bachelor's degree in Nursing preferred

• Minimum 1 year of clinical nursing experience, preferably in skilled nursing, long-term care, or post-acute care

• Previous wound care experience required

• Experience overseeing or coordinating wound care programs strongly preferred

• Wound care certification such as WCC, CWCN, CWS, or CWOCN preferred

• Strong knowledge of pressure injuries, diabetic wounds, venous and arterial ulcers, skin integrity, and wound prevention
