As a Physical Therapist Assistant, you will play a vital role in our mission to revolutionize care delivery. Reporting to the Physical Therapy Department, you will work closely with our team of licensed therapists to design and implement personalized therapy programs that promote functional independence and well-being for our patients.

Responsibilities:

• Assist licensed Physical Therapists in the development and implementation of individualized therapy plans
• Conduct evaluations, assessments, and re-evaluations of patients to identify strengths, limitations, and goals
• Implement treatment plans, including exercises, modalities, and educational programs
• Communicate with patients, families, and healthcare providers to ensure seamless care coordination
• Maintain accurate records and documentation of patient progress
• Participate in interdisciplinary team meetings to share treatment plans and goals
