As part of our dynamic interdisciplinary team, we are seeking a talented Physical Therapist Assistant (PTA) to join our Post-Acute Rehabilitation Services.

About the Role:

As a Physical Therapist Assistant (PTA) at Spring Creek Healthcare Center, you will play a vital role in providing high-quality, patient-centered care to our residents and patients. You will work under the supervision of licensed Physical Therapists to design and implement individualized treatment plans that promote recovery, independence, and overall well-being. Your expertise will be invaluable in helping us deliver exceptional care that is tailored to the unique needs of our patients.

Responsibilities:

• Assist Physical Therapists in conducting patient assessments, developing treatment plans, and implementing interventions to achieve established goals.
• Provide direct patient care, including but not limited to: exercise training, walking programs, and assisting patients with exercises and activities of daily living (ADLs).
• Communicate effectively with patients, families, and healthcare professionals to ensure a comprehensive understanding of patient needs and progress.
• Document patient care and treatment plans in accordance with our electronic medical record system.
• Participate in interdisciplinary team meetings to discuss patient care and outcomes.
• Maintain accurate and up-to-date records, including patient charts and billing information.
• Collaborate with other healthcare professionals to ensure seamless transitions and continuity of care.
