About the Role:

As a Physical Therapist (Per Diem) with Silver Healthcare, you will work on a per diem basis, providing physical therapy services to patients in our skilled nursing facility. Your duties will include:

• Evaluating and treating patients with various physical challenges, such as injuries, illnesses, or disabilities

• Developing and implementing individualized treatment plans designed to meet each patient's unique needs and goals

• Using a variety of physical therapy techniques, including exercise, manual therapy, and other modalities

• Collaborating with other healthcare professionals to ensure comprehensive care and optimal outcomes

• Maintaining accurate and detailed records of patient treatments and progress

Responsibilities:

• Provide high-quality physical therapy services to patients in a skilled nursing facility setting

• Conduct initial evaluations and ongoing assessments of patients' physical function and progress

• Develop and implement individualized treatment plans that address patients' physical, emotional, and cognitive needs

• Implement and modify treatment plans as needed to ensure optimal patient outcomes

• Communicate effectively with patients, families, and other healthcare professionals to ensure seamless care coordination

• Maintain accurate and detailed records of patient treatments and progress
