• Assisting clients with daily living activities, such as bathing, dressing, and grooming
• Providing medication management and administration
• Assisting with meal preparation and feeding
• Offering companionship and emotional support
• Maintaining accurate records of client care and services provided

Responsibilities:

• Develop and implement personalized care plans in collaboration with clients, family members, and healthcare professionals
• Identify and address client needs, preferences, and concerns in a compassionate and professional manner
• Maintain a safe and clean environment for clients, adhering to infection control and safety protocols
• Collaborate with other healthcare professionals to ensure seamless care transitions and coordination
