As an RN Case Manager, you will be an integral member of the interdisciplinary team, coordinating care and connecting patients, families, and caregivers with the resources they need throughout their hospice journey. You will collaborate with hospice colleagues,

volunteers, physicians, and community partners to implement individualized plans of care focused on comfort and quality of life.

What You Bring:

· An active Massachusetts Registered Nurse license in good standing

· Bachelor’s degree in nursing is preferred

· Hospice experience preferred

· Strong nursing skills and the ability to build supportive connections with terminally ill patients and their families

· The ability to assess, adapt, and respond to patient needs across a variety of care settings

· A valid driver’s license and auto insurance in Massachusetts
