Responsibilities:

• Provide direct patient care to residents, including administering medications, performing treatments, and documenting patient progress

• Collaborate with our interdisciplinary team to develop and implement individualized care plans

• Monitor and report changes in patient condition to the healthcare team

• Educate patients and families on disease management, treatment options, and post-discharge care

• Participate in quality improvement initiatives to improve patient outcomes and facility processes

• Maintain accurate and timely documentation of patient care and health information
