Responsibilities

• Conduct comprehensive evaluations and develop individualized treatment plans for patients with various physical therapy needs

• Deliver high-quality, patient-centered care in a mobile outpatient setting, working closely with patients and families to achieve their goals

• Implement and supervise treatment plans, ensuring effective and efficient patient care

• Collaborate with multidisciplinary teams to develop and implement comprehensive care plans

• Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination

• Maintain accurate and detailed records of patient treatment and progress

Requirements

• Current licensure as a Physical Therapist in the state of Utah

• Master's or Doctoral degree in Physical Therapy from an accredited program

• Experience working in a mobile outpatient setting preferred

• Strong communication and interpersonal skills

• Ability to work independently and as part of a team

• Familiarity with electronic medical records (EMRs) and practice management software
