We're seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our skilled nursing facility on 1st shift (7AM–3PM) with every-other-weekend scheduling. As an LPN at Carbondale Rehabilitation and Healthcare Center, you'll provide skilled nursing care that helps residents thrive throughout the day.

Responsibilities:

• Deliver direct nursing care according to individualized care plans
• Administer medications and treatments accurately and safely
• Closely monitor residents, quickly recognize changes, and communicate with RNs and physicians
• Maintain thorough, timely, and accurate documentation
• Guide and support CNAs to ensure consistent, high-quality care
• Assist with smooth admissions, transfers, and discharges
• Build trusting relationships with residents, families, and the care team
• Uphold the highest standards of infection control and resident safety
