Job Summary:

Responsibilities:

• Provide direct patient care to a diverse population, including assessments, medications, treatments, and procedures

• Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams

• Monitor and document patient progress, identifying and escalating concerns as necessary

• Participate in quality improvement initiatives to improve patient outcomes and reduce readmissions

• Collaborate with interdisciplinary teams to develop and implement evidenced-based practices

• Maintain accurate and up-to-date records, adhering to HIPAA and facility policies

• Communicate effectively with patients, families, and healthcare teams, promoting a culture of transparency and respect

Requirements:

• Current Registered Nurse (RN) license in the State of Illinois
