About the Role

We’re seeking a compassionate and detail‑oriented Start of Care Nurse to join our home health team. This role is critical in ensuring patients receive a smooth transition into care services, with thorough assessments and individualized care planning from day one.

Responsibilities

• Conduct comprehensive patient assessments at the start of care

• Develop and implement individualized care plans

• Coordinate with physicians, caregivers, and interdisciplinary team members

• Provide patient and family education on care procedures and health management

• Ensure compliance with agency policies and regulatory standards

• Document all assessments and interventions accurately and timely

Qualifications

• Current RN license in

• Minimum 1 year of nursing experience (home health preferred)

• Strong clinical assessment and documentation skills

• Excellent communication and interpersonal abilities

• Ability to work independently and manage multiple priorities

• Familiarity with EMR systems a plus
