Job Summary:

Highfield Gardens Care Center, a sub-acute and long-term care facility in Great Neck, New York, is seeking an organized Discharge Planner to join our team. As a key member of our healthcare community, you will play a vital role in ensuring a seamless transition for our residents back to their homes or alternative settings.

Discharge Planner Responsibilities:

• Coordinate with multidisciplinary teams, including physicians, nurses, and social workers, to develop individualized discharge plans for residents

• Assess residents' medical, social, and psychological needs to determine the most appropriate post-acute care setting

• Facilitate communication with family members, care providers, and other stakeholders to ensure a smooth transition

• Identify and address potential barriers to discharge, such as insurance and transportation issues

• Maintain accurate records and reports on discharge planning activities and outcomes

• Collaborate with internal departments, including admissions, marketing, and quality improvement, to ensure a cohesive approach to care

Discharge Planner Requirements:

• Bachelor's degree in a healthcare-related field (e.g., social work, nursing, healthcare administration)

• Prior experience in social work, SNF, or a related field

• Strong communication, interpersonal, and problem-solving skills

• Ability to work in a fast-paced environment and prioritize multiple tasks and deadlines
