Responsibilities:

• Provide direct patient care to our residents and patients, including assessments, medications, and treatments

• Develop and implement individualized care plans in collaboration with the interdisciplinary team

• Monitor and report changes in patient condition to the interdisciplinary team and physicians

• Participate in quality improvement initiatives to ensure high-quality patient care and outcomes

• Maintain accurate and detailed medical records

• Collaborate with other healthcare professionals to provide comprehensive care

Requirements:

• Current RN license in the state of Pennsylvania

• One year of experience as a Registered Nurse in a long-term care or rehabilitation setting

• Strong clinical skills and ability to work effectively in a fast-paced environment

• Excellent communication and interpersonal skills
