Responsibilities:

• RN MDS Coordinator: Coordinate and maintain accurate and timely MDS assessments, care plans, and care updates for Medicare and Medicaid residents.

• Collaborate with healthcare team members to develop individualized care plans that meet the unique needs of each patient.

• Review and revise care plans as necessary to ensure continuity of care and compliance with regulatory requirements.

• Maintain accurate and up-to-date records and reports, including MDS assessments, care plans, and care updates.

Requirements:
