Staff Social Worker

A skilled nursing facility in Brooklyn, NY is looking for an experienced Staff Social Worker for their sub-acute unit.

Salary: $75K to $90K range (based on experience)

Qualifications:

• Must have BSW or MSW

• Must have 2+ years experience as a Social Worker in long term care

• Must have experience in a nursing home setting

• Must have strong organizational and case management abilities 

• Excellent communication and interpersonal skills

Staff Social Worker Job Description:

• Assist new residents and their family in the initial adjustment phase, as well as in the ongoing transition to institutional living

• Provide information to the resident, and/or family member, on residents' rights and responsibilities
