About the Opportunity

Firefly Home Health and First Call Care Management are looking for a compassionate, reliable, and experienced Registered Nurse (RN) to provide services to clients throughout Lake and Porter Counties.

This dual-employment opportunity is ideal for an RN who enjoys providing hands-on in-home nursing care while also helping older adults and their families navigate healthcare needs through geriatric care management.

The selected candidate will work with two organizations:

• Firefly Home Health: Providing in-home nursing services and supporting clients’ healthcare needs.

• First Call Care Management: Assessing, coordinating, and overseeing care for older adults while supporting their families.

The position will initially be part-time or PRN, with the possibility of increased hours as the client base expands.

Please note: This opportunity is open to Registered Nurses only. Licensed Practical Nurse (LPN) applicants will not be considered.

Responsibilities

Firefly Home Health

• Provide in-home nursing visits throughout Lake and Porter Counties.

• Perform nursing assessments and identify clients’ healthcare needs.

• Observe, monitor, and document changes in clients’ health conditions.

• Deliver nursing care in accordance with individualized plans of care.

• Collaborate with clients, families, physicians, caregivers, and other healthcare professionals.

• Educate clients and families about treatments, medications, care needs, and safety practices.

• Maintain complete, accurate, and timely clinical documentation.

• Comply with applicable home health regulations, company policies, and professional nursing standards.

First Call Care Management

• Complete comprehensive assessments of clients’ medical, functional, psychosocial, and environmental needs.

• Create, implement, and oversee individualized care plans.

• Coordinate healthcare providers, home-care services, and community-based resources.

• Act as a point of contact between clients, families, healthcare providers, and other care team members.

• Advocate for clients’ preferences, safety, independence, needs, and overall quality of life.

• Guide and support families as they navigate healthcare systems and make care-related decisions.

• Monitor clients’ changing needs and update care plans as appropriate.

• Maintain accurate and current client records.

• Follow all applicable laws, regulations, ethical guidelines, and organizational policies.

Qualifications

• Active, unrestricted Indiana Registered Nurse (RN) license.

• RN applicants only; LPN applicants will not be considered.

• At least two years of experience in home health, geriatric care, case management, or a related healthcare setting.

• Geriatric Care Manager or Aging Life Care Manager certification preferred.

• Strong understanding of healthcare systems and resources available to older adults.

• Excellent clinical assessment, communication, documentation, and care-planning abilities.

• Strong organizational skills and the ability to manage multiple clients and responsibilities.

• Ability to work independently while collaborating effectively with families and healthcare professionals.

• Valid driver’s license and dependable transportation.

• Willingness and ability to travel throughout Lake and Porter Counties.

• Compassionate, dependable, patient, and professional approach to care.

Schedule

This role will begin with a flexible part-time or PRN schedule. Hours will vary depending on home health visit requirements, care management assignments, and individual client needs.

The ideal candidate will be interested in working with both Firefly Home Health and First Call Care Management and be comfortable managing responsibilities across both organizations.
