Evening LPN

Job Summary:

We are seeking a compassionate and dedicated Evening LPN to join our healthcare team at Shippenville Nursing and Rehab in Shippenville, Pennsylvania. As an Evening LPN, you will play a vital role in providing high-quality patient care and contributing to the overall success of our facility. Responsibilities:

Provide direct patient care to residents, including taking vital signs, administering medications, and performing treatments as needed

Assist with personal care, such as bathing, dressing, and toileting, to maintain the dignity and comfort of our residents

Develop and implement individualized care plans in collaboration with the interdisciplinary team

Communicate effectively with residents, families, and healthcare team members to ensure seamless coordination of care

Participate in shift report and share information with the morning LPN team

Maintain accurate and up-to-date records of patient care and medication administration

Qualifications:

Current LPN licensure in the state of Pennsylvania

High school diploma or equivalent required

Experience in long-term care or rehabilitation preferred
