Responsibilities:

• Provide individualized, patient-centered care to residents and patients, including assessment, planning, implementation, and evaluation of care plans

• Develop and implement effective treatment plans, incorporating evidence-based practices and protocols

• Collaborate with the interdisciplinary care team to ensure seamless coordination of care

• Communicate effectively with patients, families, and healthcare professionals to ensure continuity of care

• Participate in quality improvement initiatives to enhance patient outcomes and satisfaction

• Maintain accurate and detailed records of patient care, treatment plans, and progress
