Responsibilities:

• Provide direct patient care, including administering medications, performing vital signs, and assisting with activities of daily living
• Collaborate with the healthcare team to develop and implement individualized care plans
• Monitor and report changes in patient condition to the nursing team and physicians
• Participate in care conferences and treatment team meetings to ensure comprehensive care
• Demonstrate a commitment to excellent customer service and promote a positive patient experience
