Home Care Intake Coordinator

This is an in-office administrative position, not a caregiver or CNA role. No CNA license is required. You will not provide hands-on care. This role focuses on processing referrals, entering client information, verifying eligibility, completing enrollment paperwork, and coordinating follow-up.

Rockaway Home Care, a division of Foundation For the Elderly, is seeking a dependable and organized Home Care Intake Coordinator to join our St. Louis, Missouri office.

In this role, you will serve as a primary point of contact for new referrals and help consumers navigate the enrollment process for Consumer Directed Services (CDS). You will ensure client information is accurately documented, required paperwork is completed, and referrals move forward efficiently.

What You'll Do

• Receive and process new CDS referrals by phone, email, and fax.

• Enter, review, and verify client information in internal systems.

• Contact consumers and their families to explain the enrollment process and gather required information.

• Check eligibility and complete enrollment paperwork.

• Follow up with consumers, families, and referral sources to obtain missing documents.

• Maintain accurate records and ensure intake documentation is complete.

• Communicate professionally with consumers, families, referral partners, and internal team members.

• Track pending referrals and follow up to meet processing deadlines.

• Identify missing information or issues that may delay enrollment and take appropriate action.

• Help ensure a smooth and timely transition through the intake process.

What We're Looking For

• At least one year of administrative, intake, data entry, or customer service experience preferred.

• Healthcare or home care experience is a plus but is not required.

• Strong verbal and written communication skills.

• Comfortable with data entry, Microsoft Office, and learning new computer systems.

• Excellent organizational skills and attention to detail.

• Ability to manage multiple tasks and meet deadlines.

• Consistent follow-up skills and a proactive approach to problem-solving.

• Self-motivated, dependable, and able to work independently.

• Professional, courteous, and team-oriented.

Clinical experience and hands-on caregiving experience are not required. No CNA license is needed.

Compensation & Benefits
