About the Role:

As a Social Worker, you will play a vital role in supporting the emotional and psychological well-being of our residents, Our community blends beautiful spaces and services, ensuring residents thrive, build connections, and enjoy every day to the fullest. With a competitive salary range of $85,000 - $90,000 ANNUALLY, you will have the opportunity to work closely with our interdisciplinary team to identify and address the unique needs of each resident, ensuring they thrive and enjoy every day to their fullest potential.

Responsibilities:

• Conduct social work assessments to identify and address the physical, emotional, and psychological needs of residents
• Develop and implement care plans to address the identified needs of residents
• Collaborate with the interdisciplinary team to provide comprehensive care and services to residents
• Provide counseling and support to residents and their families to promote emotional well-being and independence
• Maintain accurate and timely records of resident assessments, care plans, and progress notes
• Participate in care conferences and other meetings to discuss resident care and services

Requirements:

• Bachelor's degree in Social Work or related field
• Minimum 1 years of experience in a nursing facility
• License or certification in Social Work in NJ
