Job Summary:

Bridgewater Center for Rehabilitation and Nursing is seeking compassionate and caring Licensed Practical Nurses (LPNs) to join our team on our overnight shift (10:30PM-6:30AM). As part of our dedicated medical and nursing staff, you will play a vital role in providing high-quality care to our residents. Responsibilities:

• Provide direct patient care to residents on our Long Term Care units and specialty skilled areas

• Implement individualized care plans under the supervision of the registered nurse

• Monitor and report changes in residents' conditions to the nursing team

• Participate in medication administration and management

• Collaborate with other healthcare professionals to ensure effective care delivery

• Maintain accurate and up-to-date records of patient care

• Shift differential: $7.00 shift differential for 2nd shift and $3.00 shift differential for 3rd shift
