We are seeking a detail-oriented MDS Coordinator to manage assessments and ensure accurate documentation.

Responsibilities:

• Complete and submit MDS assessments

• Maintain accurate records

• Collaborate with the care team

Qualifications:

• Active nursing license

• Knowledge on NOMNOC, Care Planning, Care Plan Meetings, BIMS and PHQs.

• MDS experience preferred

• Strong attention to detail
