Social Services Director

CareCore at Westmoreland is a skilled nursing and rehabilitation facility in the Chillicothe area is seeking an experienced and compassionate Social Services Director to join our team.

This position plays a vital role in supporting residents and families through admissions, discharge planning, care coordination, emotional support, and advocacy throughout their stay.

Responsibilities:
• Coordinate and oversee social services for residents in a long term care setting
• Assist with admissions, discharge planning, and transition of care
• Support residents and families with emotional, social, and financial needs
• Collaborate with interdisciplinary team members to ensure quality resident outcomes
• Maintain compliance with state and federal regulations
• Advocate for resident rights and overall well-being
• Participate in care plan meetings and family communication

Qualifications:
• Current LSW license strongly preffered
• Previous long term care or skilled nursing experience preferred
• Strong communication and organizational skills
• Knowledge of discharge planning, care coordination, and resident advocacy
• Compassionate, professional, and team-oriented approach
