We are seeking a dedicated and compassionate Psychiatric Rehabilitation Service Coordinator (PRSC) to join our team at Oak Park Oasis. As a PRSC, you will play a crucial role in ensuring the seamless transition of patients from hospital to home, providing high-quality care that sets us apart as a Joint Commission Approved facility.

About the Role:

In this full-time position, you will work closely with our multidisciplinary care team to develop individualized plans of care for patients with complex psychiatric and rehabilitation needs. Your responsibilities will include:

• Conducting comprehensive assessments of patients' condition prior to discharge from the hospital

• Creating tailored plans of care, monitoring progress, and communicating with patients' physicians and families

• Coordinating additional outside services to ease the transition back home and ensure continuity of care

• Collaborating with our medical director and other healthcare professionals to achieve outstanding patient outcomes

Responsibilities:

• Conduct comprehensive assessments of patients' condition prior to discharge from the hospital

• Develop individualized plans of care in collaboration with the multidisciplinary care team

• Implement and monitor patients' care plans, making adjustments as needed

• Communicate with patients' physicians and families to ensure seamless care transitions

• Coordinate additional outside services to support patients' independence and well-being

• Participate in multidisciplinary care team meetings to share knowledge and best practices
