Job Summary:

Hickory Grove Post Acute is seeking a compassionate and dedicated Evening Shift LPN to join our team. Responsibilities:

• Provide direct patient care to residents under the supervision of a registered nurse, including administering medications, assessing vital signs, and implementing care plans

• Monitor and report changes in resident condition to the nursing staff and consulting with the registered nurse as necessary

• Maintain accurate and thorough records of patient care and progress

• Collaborate with other healthcare professionals to ensure seamless care transitions and optimal outcomes

• Participate in care conferences and individualized care planning

• Maintain a safe and clean environment, adhering to infection control protocols and maintaining confidentiality

Requirements:

• Associate's degree in Nursing (ADN) or equivalent

• Current LPN licensure in the state of Tennessee

• At least 6 months of experience as a Licensed Practical Nurse (LPN)

• Strong communication and interpersonal skills

• Ability to work effectively in a fast-paced environment with multiple priorities

• Knowledge of standard nursing procedures and protocols
