Evening Shift Licensed Practical Nurse

Hickory Grove Post Acute is seeking an experienced and compassionate Evening Shift Licensed Practical Nurse (LPN) to join our dedicated team of healthcare professionals. Responsibilities:

• Provide nursing care to patients in a post-acute setting, including administering medications, performing wound care, and assisting with activities of daily living (ADLs)

• Develop and implement individualized care plans in collaboration with the interdisciplinary team

• Monitor and report changes in patient condition, including vital signs and any medication side effects

• Maintain accurate and up-to-date records of patient care and treatment

• Collaborate with other healthcare professionals to ensure seamless care transitions

• Participate in Quality Improvement initiatives to enhance patient outcomes and satisfaction

Requirements:

• Current licensure as a Licensed Practical Nurse (LPN) in the State of Tennessee

• High school diploma or equivalent required; Associate's degree or Bachelor's degree in Nursing preferred
