Job Summary:

Responsibilities:

• Provide licensed clinical social work services to patients in a long-term care and rehabilitation setting, including assessment, diagnosis, treatment, and counseling

• Develop and implement individualized treatment plans to meet the unique needs of each patient

• Collaborate with interdisciplinary teams, including psychiatrists, psychologists, and other healthcare professionals, to ensure comprehensive care

• Conduct individual and group therapy sessions, as well as family therapy as needed

• Monitor patient progress and adjust treatment plans accordingly

• Maintain accurate and thorough records of patient interactions and treatment plans

• Stay current with industry developments and best practices in mental health care
