• Provide personalized care and support to clients in their homes, following individualized care plans

• Assist with daily living activities, such as bathing, dressing, and grooming

• Manage medications and provide medication reminders

• Assist with light housekeeping and laundry

• Maintain accurate records of client care and services provided

• Communicate effectively with clients, families, and healthcare professionals

• Collaborate with the care team to ensure seamless client care
