Responsibilities:

• Provide direct patient care under the supervision of licensed nurses, including vital sign monitoring, assistance with daily living activities, and wound care
• Collaborate with the healthcare team to develop and implement individualized care plans
• Maintain patient confidentiality and adhere to HIPAA regulations
• Participate in resident-focused activities and events to promote socialization and engagement
• Perform tasks related to patient admission, transfer, and discharge, including data entry and charting
• Demonstrate a commitment to our residents' well-being and quality of life
