Responsibilities:

• Conduct thorough initial assessments and gather necessary information from new residents, families, and healthcare providers to develop individualized care plans

• Coordinate the admission process, including reviewing medical records, updating resident data, and completing necessary paperwork

• Collaborate with the healthcare team to address resident needs, concerns, and goals, ensuring a comprehensive approach to care

• Develop and maintain effective relationships with residents, families, and healthcare providers to ensure a smooth transition to our community

• Participate in interdisciplinary team meetings to discuss resident care and progress

• Maintain accurate and up-to-date records, reports, and documentation

• Stay current with regulatory requirements, policies, and procedures, and adhere to established standards and guidelines
