As a Per Diem Physical Therapist (PT) at Bradford Manor Nursing and Rehabilitation, you will have the opportunity to join a team of dedicated healthcare professionals who strive to provide quality skilled nursing and rehabilitation services to the communities we serve. As a key member of our rehabilitation team, you will play a vital role in helping patients achieve their goals and regain independence. We are seeking a highly motivated and innovative Physical Therapist (PT) to work on a per diem basis, providing physical therapy services to our residents and patients.

About the Role:

• Provide high-quality physical therapy services to patients with a wide range of diagnoses and rehabilitation needs, including orthopedic, neurological, and cardiovascular conditions.

• Develop and implement individualized treatment plans to meet the unique needs and goals of each patient.

• Conduct thorough evaluations and assessments to identify patient strengths, limitations, and areas for improvement.

• Collaborate with other healthcare professionals, including nursing staff, occupational therapists, and speech therapists, to ensure comprehensive care and optimal patient outcomes.

• Maintain accurate and detailed patient records, adhering to all relevant regulations and guidelines.

Requirements:

• Current Physical Therapist license in the state of Pennsylvania.

• Bachelor's or Master's Degree in Physical Therapy from an accredited program.

• Effective communication and interpersonal skills to work with patients, families, and healthcare team members.

• Ability to work in a fast-paced environment, prioritizing tasks and managing multiple responsibilities.
