About the Role

As a Home Health Caregiver with Homewell of Southern PA, you will play a vital role in delivering exceptional care to our clients in their own homes. Your compassion, empathy, and dedication will make a meaningful difference in the lives of our clients and their families. Home Health Caregiver responsibilities may include:

• Providing one-on-one care to clients, including assistance with daily living activities, medication management, and emotional support
• Developing and implementing individualized care plans tailored to each client's unique needs and goals
• Maintaining accurate and detailed records of client care, including progress notes and medication administration
• Collaborating with interdisciplinary team members, including nurses, therapists, and social workers, to ensure comprehensive care
• Demonstrating a strong commitment to client-centered care, dignity, and respect

Requirements

• High school diploma or equivalent required (or in the process of obtaining)

• Previous experience in the healthcare industry, preferably in a home health or geriatric setting

• Strong communication and interpersonal skills

• Ability to work independently and as part of a team

• Reliable transportation and a valid driver's license (if applicable)
