Certified Occupational Therapy Assistant (COTA) – Full-Time and/or Part-Time Home Care

Coverage Area: Northwest & Northeast Philadelphia (Community-Based)

Deer Meadows Home Health and Support Services is seeking a compassionate and motivated Certified Occupational Therapy Assistant (COTA) to join our growing home care team. This community-based position offers the opportunity to help patients maintain independence and improve quality of life in the comfort of their own homes.

Responsibilities:

• Provide occupational therapy treatments under the direction of a licensed Occupational Therapist

• Assist patients with ADLs, functional mobility, strength, coordination, and safety awareness

• Educate patients and caregivers on adaptive techniques and home safety

• Complete timely and accurate documentation using Home Care Home Base (HCHB)

• Communicate effectively with the interdisciplinary care team

• Monitor and report patient progress

Qualifications:

• Graduate of an accredited Occupational Therapy Assistant program

• Current Pennsylvania COTA license

• Valid driver's license and reliable transportation

• Home health experience preferred, but not required

• Experience with Home Care Home Base a plus

• Strong communication and organizational skills
