About the Role:

As a CNA, you will play a vital role in providing personalized care to our patients, ensuring their physical, emotional, and social needs are met. You will work closely with our interdisciplinary team to develop and implement individualized care plans, while promoting a positive and supportive environment for our patients and their families.

Responsibilities:

• Provide direct patient care, including vital sign monitoring, assistance with activities of daily living, and medication administration

• Assist with daily living activities, such as bathing, dressing, and grooming

• Maintain accurate and up-to-date records of patient care and services provided

• Collaborate with the interdisciplinary team to develop and implement care plans

• Promote a safe and infection-free environment for patients and staff

• Participate in quality improvement initiatives to ensure the highest level of patient care

Requirements:

• Current CNA certification in the state of Texas

• High school diploma or equivalent required

• Ability to work independently and as part of a team

• Excellent communication and interpersonal skills

• Ability to lift, transfer, and reposition patients as needed

• Reliable transportation to and from work
