About the Role:

Serenity Estates, a nurse-owned and resident-focused organization, is seeking a compassionate and highly organized Hospital Liaison to join our team! As a Hospital Liaison, you will play a vital role in ensuring seamless transitions between our assisted living facility and the hospital. Responsibilities:

• Coordinate with hospital staff to ensure efficient and effective communication regarding resident hospitalizations, discharges, and transfers

• Conduct thorough assessments of resident needs and develop personalized care plans in collaboration with our interdisciplinary team

• Advocate for residents' rights, dignity, and well-being, promoting a culture of respect and empathy

• Facilitate communication between residents, families, and healthcare providers to ensure continuity of care

• Participate in quality improvement initiatives to enhance the overall resident experience

• Build and maintain strong relationships with hospital staff, physicians, and other healthcare professionals

• $80,000 to 100,00K depending on education and experience.

Requirements:

• 2 years of experience in healthcare sales.

• Strong communication, problem-solving, and organizational skills

• Ability to maintain confidentiality and handle sensitive information
