Responsibilities

• Provide direct patient care to residents in our long-term care facility, including administering medications, wound care, and other treatments as prescribed

• Monitor and report resident conditions, changes, and concerns to the nursing staff and physicians

• Assist with daily living activities such as bathing, dressing, and grooming as needed

• Participate in developing and implementing individualized care plans for each resident

• Collaborate with other healthcare professionals to ensure comprehensive care

• Maintain accurate and timely records of resident care and treatments

Requirements

• Current LPN license in the State of Pennsylvania

• High school diploma or equivalent required

• 1+ year of experience as an LPN in a long-term care setting preferred
